Because our bags are intended for children currently facing pediatric cancer, we ask for a simple attestation and an optional contact we can reach out to. No medical records or documentation required.

I confirm that this child is currently undergoing or recovering from pediatric cancer treatment, and that I am authorized to request a bag on their behalf.

We may reach out to confirm eligibility, but this is entirely optional and never required.
